Health Equity is a fundamental component of quality. An organization can achieve the best outcomes by providing equitable care to every patient receiving the right care at the right time, and in the right way, according to unique needs including racial, ethnic or linguistic background. This presentation will highlight strategies on cultural tailoring to improve culturally responsive, patient-centered care in the medical home. (UAN 0850-0000-17-003-L03-P)
