The goal of this presentation is to provide insight and guidance to practices on how to engage vulnerable patients through the provision of health information that is accessible, understandable, and actionable to these patient populations. Participants will learn about the latest research findings on the barriers experienced by vulnerable patient populations as they attempt to engage with the health care system and actively participate in their care. Examples presented will feature the work of safety-net primary care practices including how they approached these efforts with limited resources, as well as the challenges and successes experienced through their transformation.
