The demands on healthcare professionals to provide quality healthcare is increasing as we move into an era of quality-driven models of care. The resources needed to meet these demands are often high and the specifics on how to meet these demands are broad and general and often not specific to any one discipline. This can leave healthcare professionals trying to meet the demands set forth for them with limited resources and inadequate education. Pre-visit planning is a process that is flexible and conducive to many environments and can be tailored to fit the needs of any healthcare discipline. This session will present effective ways to use pre-visit planning for patient encounters in order to achieve improvement in quality measures, Annual Wellness Visits, patient engagement, and overall PCMH sustainability.
