A PCMH-FOCUSED AGENDA Welcome to the 2017 Patient-Centered Medical Home Congress. This is a can’t-miss conference for those who want to be part of an expanded educational program focused on the journey of transformation and recognition success. Attend specialized tracks and network with high-level executives throughout the care delivery system.
Public and private payers are moving away from fee-for-service (FFS) payments to alternative payment and care delivery models that promote the triple aim. Although the evidence base for successful alternative payment models (APMs) is still emerging, Massachusetts' Primary Care Payment Reform initiative was foundational and provided many lessons in the design and implementation of newer APMs. (UAN 0850-9999-17-007-L04-P)
