The PCMH and the health center model share the same vision of high-quality, cost-effective care that is tailored to their patient population's needs and focused on improving health outcomes, yet FQHCs often do not feel confident they can achieve such formal distinction. However, because these models share common goals and UDS echoes multiple factors within PCMH measures, health centers often have an advantage over other organizations in pursuit of PCMH recognition. This session will share our knowledge and processes to demonstrate to other health center practices that the transformation they are hesitant to pursue is within their reach, as these principles have been embedded in their model of care for over 50 years.
