Limited access to primary care services has reached a critical point in care delivery that threatens to undermine efforts for population health. Primary care providers report burnout at record numbers. As such, a health system aunched a Community Care Consortium to assist both employed and affiliated primary care practices in developing innovative, comprehensive, and coordinated solutions for patient-centered, team-based care delivery. This partnership includes patients, providers, staff, administrative, and community leaders and is piloting various care transformation initiatives, including an expanded care team model with care guides and health advisors.
