This session will share lessons learned from Battleboro Memorial Hospital’s participation in a two-year, state-wide learning collaborative focused on community-based care management of patients with complex needs. Participants will learn how this interplays with NCQA's care plans and quality initiatives, as well as the positive impact this work has on influencing change among patients experiencing these issues and patients with chronic conditions. Learning Objectives
  • Identify tools that are effective in engaging patients and community partners in a collaborative approach to care management
  • Employ SBIRT (Screening, Brief-Intervention and Referral to Treatment) into your Medical Group Practice or Emergency Department
  • Choose quality initiatives, chronic conditions for care plans and preventive measures that satisfy NCQA and ACO’s quality metrics
