This session will share lessons learned from Battleboro Memorial Hospital’s participation in a two-year, state-wide learning collaborative focused on community-based care management of patients with complex needs. Participants will learn how this interplays with NCQA's care plans and quality initiatives, as well as the positive impact this work has on influencing change among patients experiencing these issues and patients with chronic conditions.
