This session’s panel represents a collaborative effort by a Federally Qualified Health Center (FQHC) and a multi-payer PCMH initiative, the Care Transformation Collaborative of Rhode Island, to design and implement a unique clinical transformation program. FQHC organization leadership and PCMH practice facilitators from the Brown Primary Care Transformation Initiative will share successes, challenges, and opportunities for replication of their value-driven effort. Learning Objectives

  • Describe how PCMH practice facilitators can collaborate with primary care practices to help them implement practice transformation efforts so that they are successful and sustainable

  • Identify tools, resources, and strategies for transformation projects and how they can be used at different stages of clinical transformation

  • Identify common barriers and solutions for identifying transformation leadership, engaging staff and providers, adopting PCMH change concepts, and measuring outcomes throughout the clinical transformation process
