In order to address disparities in infant mortality, Mountain Area Health Education Center (MAHEC) implemented the Community Centered Health Home model and partnered with African American women in a low-income housing community. Learn how, after two years, MAHEC clinical leaders and quality improvement staff shifted the practice culture to be more aware of health disparities, social determinants of health of low-income women, and how to support healthier pregnancy outcomes for the most vulnerable.
