This session will introduce practices who are currently recognized or pursuing recognition to the intent of PCMH Standards (and select factors within) and tools that will allow them to be sustainable within a Value Based Care delivery model. Participants will leave with a better understanding of how to utilize their PCMH recognition and demonstrate value. The interconnectivity of the PCMH Standards will be demonstrated through the implementation of practical tools. (UAN 0850-9999-17-023-L04-P)

Learning Objectives

  • Review how to use PCMH to create your Value Based Statement
  • Discuss how to remove yourself from "Standard Silos" and integrate the intent of the Standards to demonstrate improvement
  • Implement practical tools that will assist in truly transforming and practicing the PCMH principles and ultimately be sustainable in a value based system
