NCQA highly recommends the use of self-assessment and gap analysis as a baseline for understanding where a practice is on Day 1 and recognizing opportunities for transformation. This session will clearly detail the process to complete an in-depth practice assessment to identify a practice's readiness, outline a process to provide a comprehensive gap analysis based on the assessment findings, and as a result of the findings, learn techniques to prioritize the gap areas with clear and direct remediation goals. Learning Objectives

  • Assess the current processes of a practice in relationship the NCQA Patient-Centered Medical Home 2017 Standards
  • Measure the assessment outcomes and provide feedback on priorities for implementation, identification of care management populations, and improvement on the overall operation of a practice
  • Prioritize the needs of a practice and provide recommendations designed to assist the practice in achieving PCMH Recognition through workflow improvement, policy development, reporting, and EHR optimization
