Achieving PCMH recognition can be a long journey, with many barriers and detours along the way. The goal of this session is to provide team-based, practice-level guidance for developing an overall planning and approach to the patient-centered transformation journey. Understanding the importance of developing internal governance, as well as having contingent team members, champions or project participants in place, can be key factors to ensure continued momentum toward PCMH transformation.
