Newer Medicare codes, Annual Well Visits and CCM reimburse for services delivered by nurses under physician supervision, providing sufficient revenue for expanded medical home teams. This presentation will describe practical considerations for implementing these nurse services, including business cases with ROI and various models for implementation.
