Dr. Alvia Siddiqi serves as Medical Director with Advocate Physician Partners (APP).  She oversees the APP Process Improvement department, which includes a focus on clinical and value transformation projects, including practice transformation for nearly 200 aligned primary care practices to Patient-Centered Medical Homes (PCMH).  Her responsibilities also include government programs, quality, and operational oversight of the Advocate/Meridian partnership plan for nearly 100,000 Medicaid TANF beneficiaries, outpatient care management, and APP’s Community Health Workers program.  She is a member of the APP Quality Improvement, Utilization Management, Peer Review, and Clinical Integration Design committees.   Dr. Siddiqi is a Board-certified Family Medicine physician and Fellow of the American Academy of Family Physicians (AAFP).  She is an appointed member of the AAFP Commission on Quality and Practice and NCQA Patient-Centered Specialty Practice (PCSP) Advisory committee.  She also serves on the NQF Medicaid Innovation Accelerator Program.  She is Board Chair of the Illinois Academy of Family Physicians (IAFP) and serves on several state and national committees, including the CMS Advisory Panel on Outreach and Education (APOE), CMS MACRA Panel on Cost measures, and state Medicaid Advisory Quality Care Subcommittee.
