Megan Reyna, MSN, RN Director of Process Improvement Advocate Physician Partners (APP)  Megan is the Director of Process Improvement with Advocate Physician Partners (APP).  The APP Process Improvement department focuses on clinical and value transformation projects, include the Patient-Centered Medical Home program, in order to assist the system with achieving national quality and financial targets.  The APP PCMH program has supported nearly 200 primary care practices with achieving Level 2 & Level 3 NCQA PCMH recognition. Her responsibilities also include government program initiatives, including MACRA support and sustainment. Megan is a Registered Nurse by background and received her MSN from University of Illinois Chicago.
